This course focuses upon the role of the multi-disciplinary team in supporting patients with physical and mental health co-morbidities, in primary care and community settings. Importance is placed on a patient centred approach, promoting involvement of the individual and their family in care planning, with a focus on common conditions including anxiety and depression, and evaluation of a range of pharmacological, non-pharmacological, and self-management strategies.

In addition, it equips the learner with a broader understanding of the multi-systems approach to disease management and strategic service development. This course explores the pathophysiology, diagnosis and management of heart failure. It focuses on both pharmacological and non-pharmacological management of heart failure and how this may be impacted by co-morbid conditions.

Study Type Supported online learning Duration Three months Level Level 7 (postgraduate) Start Date Various - see note on registration below This module critically reviews current approaches, research, and treatments in relation to different types of dementia and the disease trajectory, including end-of-life care. The module is grounded in person-centred approaches, emphasising the importance of excellent communication and ethical decision-making skills. Current key legislation and local and national drivers will be explored, and students will also critically review the interface of health and social care and collaboration across the multi-disciplinary team.
