This module supports students in providing preventative and individualised care for people with frailty, enhancing their knowledge in relation to current treatment, management, and risk avoidance. It explores key legislation and local, national, and international drivers, and their application to the planning and delivery of evidence-based care. The module emphasises the importance of excellent communication skills and collaborative working.

Transforming Dementia Care

The module is grounded in person-centred approaches, emphasising the importance of excellent communication and ethical decision-making skills. Current key legislation and local and national drivers will be explored, and students will also critically review the interface of health and social care and collaboration across the multi-disciplinary team.
